We are looking for passionate and caring individuals to make a difference in our clients lives by proactively partnering with them to manage care plans to enable continued independence and a high quality of life in their community. You will take ownership of a caseload and have experience with Homecare packages within Aged Care.
About the Role:
Provision of quality personalised care to our valued aged care clients in their own homes through services such as:
- Ensuring quality care planning and budget management is provided to all clients as per the Aged Care Quality Standards and program guidelines by undertaking person-centred care planning
- Working effectively with internal and external stakeholders to deliver services that meet client changing needs
- Working in a safe manner, closely following all policies and procedures and ensuring compliance with quality standards
To be successful in this role you’ll need:
- A genuine passion for providing high quality care to the elderly
- Drivers license and a reliable insured vehicle
- Case Management experience including budgets
- Strong communication skills and a positive attitude
- Sound computer skills especially Microsoft Office products and ability to pick up new systems
- Willingness to apply for a Police Check
