About the position We are seeking a dedicated and compassionate Field-Based Licensed Practical Nurse (LPN) to join our dynamic team. In this role, you will provide essential nursing care in a variety of settings, including patients' homes and community-based environments. Your focus will be on delivering high-quality care, monitoring patient progress, and collaborating with interdisciplinary teams to support mental health treatment plans. The role is 30 hours per week from 9:30 AM to 4 PM, Monday through Friday. Responsibilities Deliver nursing care in the field, including medication administration, monitoring vital signs, and providing therapeutic interventions as outlined in care plans. , Conduct comprehensive assessments of patients' mental health status, track progress, and report any significant changes to the supervising psychiatrist or healthcare team. , Collaborate with healthcare professionals, including psychiatrists, social workers, and care managers, to develop and implement individualized care plans. , Provide education to patients and their families about psychiatric conditions, medical conditions, treatment options, and medication management. , Maintain accurate and timely documentation of patient interactions, care provided, and any changes in patient status. , Respond to emergency situations with appropriate interventions and support, coordinating with crisis teams as needed. , Adhere to all regulatory and agency standards, including safety protocols, confidentiality requirements, and infection control procedures. , Participate in community outreach and education efforts to promote mental health awareness and reduce stigma associated with psychiatric conditions. Requirements Completion of a Practical Nursing program and possession of a valid LPN license. , At least five years of experience as an LPN, with previous experience in psychiatric or mental health nursing highly preferred. , Strong clinical assessment skills, excellent communication abilities, and proficiency in electronic health records. , Ability to work independently and manage time effectively in diverse and sometimes challenging environments. , Valid driver's license and reliable transportation required. Nice-to-haves Additional certifications in psychiatric nursing or mental health care are advantageous. , Familiarity with Google Suite (Gmail, Google Docs, etc.). , Phlebotomy skills. , Administration of medications, including intramuscular (IM) injections.
