We're seeking a compassionate and skilled Community Health Worker to join our team. This is a remote position offering a competitive salary of $28-$35 per hour. What You'll Do: Conduct engaging, person-centered home visits to connect with Aeetna members. Educate individuals and families about health conditions, disease prevention, and self-management strategies. Build and maintain strong relationships with members, healthcare providers, and community resources. Identify and address social and economic barriers to healthcare access. Facilitate referrals to appropriate community resources and support services. Assist with medication adherence and promote healthy lifestyle choices. Collaborate with a team to enhance care coordination and improve health outcomes. What You'll Need: Completed endorsed Community Health Worker (CHW) certification program (within the last year). Proficiency with computers, including navigation, Microsoft Office Suite (Word, Excel, SharePoint), and various online systems. Ability to travel up to 70% of the time to various office and home locations within Ohio using a personal vehicle. Valid driver's license, vehicle insurance, and reliable transportation. Flexibility to work in varying environments and adapt to changing circumstances. Excellent communication, interpersonal, and organizational skills. Experience in a community-based setting or with vulnerable populations is highly preferred. Knowledge of health insurance (Medicaid) and community resources is a plus.
